Does your daughter have any medical conditions that will prevent her from fully participating in any activities throughout the course of the camp? (If yes, please explain below).
Liability*
- I understand that with my daughter's participation in Elite Ringette Training and its related activities and events, I acknowledge, appreciate and agree on the following:
1. Risk of injuries from the activities involved in this program are possible. I knowingly and freely assume all such risks, both known and unknown. Even if arising from the negligence of others in the program.
2. I release and hold harmless, Elite Ringette Training and it's organizers, officials, employees, owners of the premises used for the program, and other participants, with respect to all injuries, loss or damage to person or property, whether caused by negligence or otherwise.
* There will be First Aid trained staff in attendance. However, we suggest a parent remains on site if there are any medical concerns.

* WE ARE NOT AN AFFILIATE OF ANY PROVINCIAL SPORT ORGANIZATION. WE ARE PRIVATELY OPERATED AND INSURED.

*We will be operating our 2021 Programs in line with provincial guidelines and programs are subject to change.
